after finding vulnerable ports...what next?

I have scanned a range of IP's (used NetBrute), found the
vulnerable IP I was looking for. After that, I scanned
the IP and found that there was a list of vulnerable ports
(used PortScan). Some of them were 13, 21, 23, 25, 53,
80, 110, 119, and 443. Now what exactly does it mean when
it lists these ports? Does it mean that this IP has an
FTP server and web server running that one could connect
on, or why are these ports listed? I am concerned that
someone could possibly connect to this computer on one of
these ports, but am not sure how they would or if they
could at all. Thanks.
